Part 1: Carry out a risk assessment
Either:
- select a risk assessment that you recently carried out at work
OR - choose a machine, work process or other operational system and conduct a new risk assessment on it.
Use workplace documents to perform and submit a risk assessment for your selection above.
Make sure that you have implemented the recommended control measures in your risk assessment, or at least have attempted to implement them, before proceeding to Part 2.
Part 2: Describe the control measures
Draw up a separate list of the control measures specified in your risk assessment. Beside each one, comment briefly on how it was implemented. Also note whether it was a new control specifically designed for a new hazard identified in this risk assessment, or an existing control already in place.
For example, if you carried out a risk assessment on a new machine and identified 'flying particles' and 'loud noise' as two hazards, your control measures might have included 'safety glasses' and 'hearing protection' for all operators. If you already had a site policy for everyone in the workplace to wear safety glasses and hearing protection, then these control measures would have been 'existing', because they were not introduced specifically for this machine.
On the other hand, if your hazard was 'workers at risk of stepping over the edge of a first-floor landing', your control measures might include 'erect a tubular steel handrail barrier across the opening'. This would be a 'new' control measure, requiring steel fabrication, on-site installation, painting, signage, and so on.

Part 3: Monitor the effectiveness of the control measures
Choose one of the new controls (or a related set of new controls) that you have listed in your table. Describe how you are monitoring its on-going effectiveness. Make sure you include any monitoring methods that you plan to use, even if you haven't yet implemented them. You may wish to divide these methods into categories, such as: 'documents consulted'; 'people consulted'; and 'actions taken/observations made'.
